Our Approach to Care

Our geriatric physicians provide care for a wide range of physical and emotional limitations common in older adults, including problems such as:
- Depression
- Medication Management
- Nutrition/Malnutrition
- Pain management
- Arthritis
- Diabetes
- Atrial Fibrillation
Our geriatric physicians and nurse practitioners emphasize multidisciplinary inpatient and outpatient assessment involving a wide spectrum of health care providers, including social work, pharmacy, occupational and physical therapy, nutrition, nursing and medicine.
In addition to board-certified geriatric physicians, UW Health's expert internal medicine and family practice physicians can coordinate your care with specialty physicians - from diabetes specialists to world-renowned heart and vascular care physicians. And UW Health's Transitional Care program helps patients and families make a smooth transition between health care settings and home.
